Advance Care Planning/Advance Directive Completion with Don Freeman

Don Freeman, Program Coordinator of the Advance Care Planning Program will be leading an Advance Care Planning workshop. We will explore what is Advance Care Planning (ACP) and what are the steps necessary to complete an Advance Directive (AD)? The goal is to have written directions that ensure that your loved ones and health care professionals know what you want in a medical emergency/illness when you are unable to speak for yourself. Come learn and ask questions in a supportive space.
