In this workshop we will explore what is Advance Care Planning (ACP) and what are the steps necessary to complete an Advance Directive (AD)? The goal is to have written directions that ensure that your loved ones and health care professionals know what you want in a medical emergency/illness when you are unable to speak for yourself.
